Every request reaches us with great compassion. However, our foundation is obliged to support only those families who have fallen into financial hardship due to their child’s cancer diagnosis.

Option 1

Fill in the application form online

Open the online form in your browser, fill it out on your computer, upload additional documents and send it off.
The online form is available in German only.

Applications may be submitted in German, English or French – preferably in German.

Only fully completed applications can be accepted. All required supporting documents must also be uploaded:

  • Medical report / diagnosis
  • Tax assessment / tax return
  • Proof of income / salary statement
  • Additional supporting documents

Option 2

Upload application digitally (please do not send by e-mail)

Open the PDF form in your browser, save it locally, fill it in on your computer, save it and upload it securely to our server using the upload function.

2. Upload
Please use the upload function provided here to submit your documents digitally.

  • Medical report / diagnosis
  • Tax assessment / tax return

Send in by post

Open the PDF, print it out, fill it in and send it in.
