Declaration of Consent for the Application for Support Benefits pursuant to Art. 9, 13, 14 GDPR

Data Processing

We collect personal data of parents, children, and young people in need of support to evaluate their application for support, and request their consent by signing this form.

For this purpose, the following partly sensitive categories of data pursuant to Art. 9 GDPR are retained in our data records. The data is collected directly from the data subjects, will not be forwarded to third parties, and will be stored for at least ten years to comply with statutory requirements.

Personal Data
Of children and parents: Last name, first name, date of birth, gender, bank details, email address, family and financial background, support amount.

Health Data
Of children: Diagnosis, description of illness, health status.

Additional Information
Applicant, use of funds, remarks, board decision.

Your Rights

Right of Access

You have the right to obtain access to the data we store about you.

Rectification, Erasure, Restriction

You have the right to rectification, erasure, restriction of processing, and the right to data portability regarding your data stored by us. Due to the statutory retention period of ten years, erasure of your data is only possible after this period has expired.

Right to Object

You have the right to object even after signing this consent, without affecting the lawfulness of processing based on consent before its withdrawal.
